Before You Self-Schedule
- Please verify your benefits with your insurance provider before your visit (or call our office).
- HMO plans typically require authorization and a referral from your primary care provider prior to scheduling.
- Many medical insurance plans do not cover routine vision exams or glasses prescriptions (but will cover exams if there is a medical issue). Vision insurance typically covers routine vision exams and glasses.
- Typical self-pay fees range from approximately $150–$350+ depending on complexity and if additional testing is needed.
- If you have any questions or concerns please call (817) 529-9949 to schedule.
